
Why hip pain tends to worsen at night
For many people with hip problems, the day is manageable. Movement keeps things tolerable — muscles engage, body weight shifts, and the brain stays busy with other signals. Lying down strips all of that away. The distraction of activity disappears, protective muscle activation drops off, and the joint settles into a fixed position for hours at a time. Pain that was background noise during the day moves to the foreground.
Position compounds this. Lying on the affected side presses directly onto a joint surface or soft tissue that is already inflamed or worn. In hip osteoarthritis, cartilage loss means that lying down concentrates load on the damaged area with nothing to absorb or distribute it — one reason the ache tends to peak in the small hours rather than after a walk. With trochanteric bursitis, the same principle applies to the inflamed bursa on the outer hip.
Rest pain — pain that arrives without movement, purely at night — matters diagnostically because it narrows the picture. Mechanical problems that only hurt when you move rarely wake people at 3am. Persistent rest pain points more firmly toward inflammation, degeneration, or occasionally something that needs urgent attention.
Where the pain sits, and what it actually feels like, are the first clues to what is driving it.
The main causes — and what each feels like
Three broad patterns account for most nocturnal hip pain, and location is the fastest way to tell them apart — even before any scan.
Outer hip and thigh. Pain on the side of the hip that worsens when lying on that side typically originates in the soft tissues rather than inside the joint itself. Trochanteric bursitis and gluteal tendinopathy — inflammation of the fluid-filled sac or tendons over the bony prominence at the top of the thigh — are the usual culprits. The ache sits on the surface rather than deep inside, and direct pressure on the outer hip is often the main trigger.
Deep groin or front-of-thigh ache. Pain felt inside the groin, or spreading into the front of the thigh, points toward the hip joint itself. In older adults, this pattern is most consistent with hip osteoarthritis — typically a dull, progressive ache that stiffens in the morning or after sitting for any length of time. In younger or more active patients, a labral tear can produce a similar deep nocturnal ache, often sharper and accompanied by clicking or catching during the day.
Buttock or leg pain. When pain radiates into the buttock or travels down the leg — particularly below the knee — the hip joint is often not the source. Compression of the sciatic nerve or referred pain from the lumbar spine can closely mimic hip symptoms at night.
Position-only pain. If discomfort disappears completely on changing sleep position and causes no trouble during the day, a simple mechanical or soft-tissue irritation is the most likely explanation.
None of these patterns confirms a diagnosis on its own, but identifying where the pain sits meaningfully narrows the differential before any imaging is considered.
Hip OA vs a labral tear — how the symptoms differ
During waking hours, hip OA and a labral tear often feel quite different — and that contrast matters, even if it partly dissolves at night.
Hip OA typically develops gradually in people over 50. The discomfort is a dull, grinding ache that stiffens after rest and may ease slightly with the first few minutes of gentle movement. Morning stiffness lasting more than 30 minutes is a useful clinical marker. As OA progresses, there is a gradual, often symmetrical loss of internal rotation and flexion: shoes become harder to put on, crossing the legs grows uncomfortable.
A labral tear tends to affect younger or more active patients — often alongside femoroacetabular impingement (FAI), where a cam or pincer-shaped bone lesion repeatedly catches the labrum during hip flexion, pivoting, or deep squatting. The pain is frequently described as sharp rather than dull, and many patients report a click, lock, or catching sensation that is reproducible with certain movements. Range of motion can remain near normal for considerably longer than in OA.
These distinctions are real and clinically useful during the day — they give a clinician important direction from the first consultation. At night, however, they become less reliable. When the affected hip is loaded by lying on that side, both OA and a labral tear can produce the same deep groin ache, with similar intensity and character. Symptom alone, in the small hours, is not a dependable separator.
Imaging does not fully resolve the ambiguity. MRI and MR arthrography — the standard tools for labral assessment — show sensitivity of only around 54–55% even in specialist centres, particularly in structurally complex hips. A normal scan does not rule out a labral tear. Clinical examination — provocation tests, careful range-of-motion assessment, and gait observation — remains essential, and in many cases carries more diagnostic weight than the imaging report alone.
Red flags: symptoms that need same-day or urgent assessment
Most nocturnal hip pain has a mechanical or degenerative cause and is not dangerous — but a small number of presentations require action the same day.
A hot, swollen joint with fever or feeling systemically unwell should be treated as a medical emergency. Septic arthritis can destroy a joint within hours; it cannot be excluded without same-day assessment, and suspected cases should go to A&E.
New hip pain after a fall, particularly in an older person who is struggling to bear weight, is a fracture until proven otherwise. Do not wait to see whether it settles — attend A&E.
Unexplained weight loss, drenching night sweats, pain that does not ease in any position, or a known history of cancer are systemic red flags that raise concern for bone malignancy or other serious pathology. Contact a GP the same day or call 111.
Hip pain in a child or adolescent with a limp, fever, or refusal to put weight through the leg always warrants same-day paediatric review.
These presentations are uncommon. When any of them applies, however, the consequences of delay are serious — seek assessment that day rather than waiting for a routine appointment.
When to see a GP — and when to escalate to a specialist
The practical question most patients arrive at — how long should this be tolerated before seeking help? — has a reasonably clear answer for nocturnal hip pain specifically.
See a GP if pain is disrupting sleep, is worsening or recurring, has not improved after two weeks of home management, or if morning stiffness on waking exceeds 30 minutes. While waiting for that appointment, sleeping with a pillow between the knees reduces lateral hip loading and can limit nocturnal discomfort; over-the-counter analgesia may help in the short term. These are holding measures, not substitutes for assessment.
Escalate to a specialist if nocturnal pain persists beyond a few weeks despite GP-level conservative management, if functional decline has developed — a new limp, difficulty dressing, or trouble managing stairs — or if imaging arranged at GP level has not resolved the diagnosis. A 2026 study of 100 patients with advanced hip OA found that those who experienced frequent night pain were roughly twice as likely to report severely disrupted sleep compared with patients whose nocturnal symptoms were less frequent. Disrupted sleep at this level is a recognised clinical signal, not simply a comfort issue, and warrants assessment rather than continued watchful waiting.
Younger patients — broadly those under 50 — merit earlier specialist input. When night pain accompanies mechanical symptoms such as clicking, catching, or pain provoked by hip flexion and rotation, the labral/FAI differential requires provocation testing and clinical examination that a GP appointment is rarely configured to deliver in full. Waiting several months on conservative measures in this group risks missing a structural diagnosis that tends to respond well to timely intervention.

The process begins with a detailed history: pain location, onset, character, how it behaves through the night and on waking, and which activities worsen or relieve it. Clinical examination follows, using gait analysis, passive and active range-of-motion testing, and targeted provocative manoeuvres — impingement tests that stress the labrum and soft-tissue palpation protocols for the trochanteric structures — to narrow down whether the source is intra-articular, periarticular, or referred from the lumbar spine. Imaging is requested after examination, not as a substitute for it: X-ray assesses structural alignment and joint-space narrowing; MRI or MRA provides soft-tissue and labral detail. Both are interpreted against the clinical findings rather than as standalone verdicts — a principle that matters particularly for labral assessment, where imaging sensitivity even in specialist settings is limited.
The practical result of working through this process is a targeted treatment recommendation. A periarticular source points toward physiotherapy-led load management and, where appropriate, image-guided injection. An intra-articular diagnosis — OA or labral pathology — opens the joint-preservation pathway, ideally at a stage when structural change is still modifiable. A referred finding redirects management toward the spine, avoiding unnecessary hip-directed intervention.
