The procedure is short, but the regeneration plays out over months. Here is what happens at each stage.

MRI imaging reviewed at consultation to confirm the cartilage defect, its size, location and the surrounding cartilage. The number of ChondroFiller® boxes is decided here.

Deliver the scaffold
A 30 to 45 minute outpatient appointment, no general anaesthetic, no surgical incision. The acellular collagen gel is delivered precisely into the defect under real-time ultrasound guidance.

Scaffold sets in minutes
The collagen gel sets in situ within minutes, filling the defect and forming a three-dimensional matrix that protects the area and invites the body’s own cells in.

Cells migrate, cartilage rebuilds
Over six to twelve months, the body’s own resident cells migrate into the scaffold and lay down hyaline-like cartilage. Improvement typically begins at six to twelve weeks, with continued benefit building over the year.

Who is ChondroFiller® for?
ChondroFiller® is appropriate across the spectrum of cartilage wear, from a contained focal defect to diffuse surface wear across the whole joint. There is no upper age limit and no defect-size limit on the injection, and it is particularly valuable as a pre-replacement option for active patients in their 60s, 70s and beyond. Below is the short list; see the suitability page for the full assessment framework.
Good candidates may include
- Cartilage wear in the joint, whether a contained focal defect or diffuse surface wear (no defect-size limit)
- Patients of any age, including active patients in their 60s, 70s and beyond
- A pre-replacement pathway for patients told they need a joint replacement who want to try preservation first
- Post-injury cartilage damage (chondral lesion, OCD, post-meniscal injury)
- Stable joint mechanics (no untreated ligament instability or major malalignment)
- Realistic expectations about a 6 to 12 month timeline for new tissue to form within the scaffold
- Knee, hip, shoulder, ankle, elbow, foot, hand or wrist (joint-agnostic)
Where caution is needed
- A single contained focal defect in an otherwise healthy joint may be a clearer fit for Liquid Cartilage™ surgical regeneration
- Untreated joint instability or major malalignment
- Active infection (joint or systemic)
- Unrealistic expectation of immediate or guaranteed pain relief

Does ChondroFiller really regenerate cartilage?
ChondroFiller is an acellular collagen scaffold that fills a cartilage defect and provides a framework for the body’s own cells to migrate in and regenerate hyaline-like cartilage. Published outcome data shows +30 IKDC in knees, +33 Harris Hip Score in hips, and MOCART MRI scores of 70 to 87 indicating high-quality cartilage regeneration, with over 19,000 global cases supporting safety. Outcomes depend on defect size, location, surrounding cartilage quality and patient suitability.
Is ChondroFiller available on the NHS or private medical insurance?
No. ChondroFiller is not routinely funded by the NHS and is not currently covered by the major UK private medical insurers, including Bupa and AXA. Patients access ChondroFiller privately on a self-funded basis at London Cartilage Clinic. We can provide an itemised invoice if you wish to submit a claim independently.

How long does ChondroFiller take to work?
ChondroFiller is a regenerative scaffold, not an immediate pain-relief injection. Most patients begin to notice improvement at six to twelve weeks, with continued benefit building over six to twelve months as new tissue establishes within the scaffold. The aim is to support natural cartilage regeneration and preserve the joint, not to mask symptoms.
What is the ChondroFiller injection procedure like, and what is the recovery?
ChondroFiller is delivered as a 30 to 45 minute outpatient injection under ultrasound guidance, with no general anaesthetic and no surgical incision. The gel sets within minutes inside the cartilage defect. Most patients walk out the same hour and return to desk work the same day or the next day, with high-impact activity paused for two to four weeks. Onward physiotherapy is optional, quoted separately.
