These figures come primarily from manufacturer-sponsored data, and that context matters for interpretation. The CER draws on a pooled set of clinical studies rather than a single independent randomised controlled trial, so the numbers should be read as the best currently available evidence rather than definitive proof.

Independent corroboration comes from the Jerosch et al. prospective post-market clinical follow-up (PMCF) study, which reported no device-related serious adverse events over a three-year follow-up period. The same study recorded a mean IKDC improvement of 32.4 points sustained at three years — a meaningful functional gain alongside the clean safety signal.

Two evidence gaps are worth naming plainly: long-term safety data beyond three years remain limited, and independent randomised controlled trial data have not yet been published. The safety picture described here reflects what the current body of evidence supports — no more.

What ChondroFiller is made from

ChondroFiller® is derived from murine Type I collagen — that is, collagen sourced from mice. This is a material fact that should be raised and documented before any injection takes place. For most patients it carries no practical consequence, but anyone with a known sensitivity to rodent or animal proteins needs to disclose that history at their pre-injection consultation, since a theoretical immunogenic risk exists. The published clinical data have not prominently flagged adverse allergic events, yet proactive screening remains the responsible standard given the biological origin of the material.

The product is acellular: it contains no donor cells of any kind. Rather than delivering cells directly, the scaffold works by providing a chemotactic environment that draws the patient's own progenitor cells — migrating in from the surrounding synovium and subchondral bone — into the matrix, where they can differentiate and lay down new tissue. It becomes a structured moment for distinguishing slow responders, still within the biological repair window, from genuine non-responders who may need reassessment and a different clinical decision. That review also anchors what a genuine non-response looks like, which matters practically: a minority of patients do not achieve the expected level of cartilage regeneration, and surrounding cartilage quality, defect characteristics, and untreated mechanical instability all influence individual outcomes. The evidence-supported benefit profile is real, but it is not universal, and patients benefit from holding that honest framing from the outset.

Factors that affect outcomes — and who may not be suitable

Pre-treatment imaging review sets the clinical conditions for safe, appropriately dosed treatment — it is not an administrative step or a cost-generating formality.

The dosing is directly MRI-dependent. ChondroFiller® is supplied in 2.3 mL boxes, and the treating clinician selects one, two, or three based on defect dimensions measured on pre-treatment imaging. A consultation that produces a price estimate without having reviewed current MRI cannot accurately specify how many boxes are required — which means it cannot accurately specify the treatment itself.

The more clinically significant finding from that imaging assessment is whether mechanical instability is present. Ligament insufficiency or joint malalignment left untreated before injection is the single factor most consistently identified in published data as undermining ChondroFiller® outcomes; it functions in practice as a contraindication. The collagen scaffold cannot compensate for abnormal joint loading, and placing it in an unstable environment risks poor progenitor-cell migration and inadequate scaffold remodelling.

Outcome variability also reflects defect location and surrounding cartilage quality. The strongest evidence base sits with the knee — the Jerosch PMCF series followed knee patients to three years with defined functional outcome measures. For the ankle, hip, and small joints, published series are smaller and follow-up periods shorter. That matters practically: for joints other than the knee, realistic expectations should be discussed individually in consultation, since it is not safe to assume that knee-derived outcome figures transfer directly to a different joint anatomy or defect pattern.

Where combination with another intra-articular therapy is being considered, the rationale and sequencing should be agreed with the treating clinician rather than assumed as a standard add-on.
